Job Description Summary:

- Conducts comprehensive health assessments, including physical examinations, patient histories, and interpretation of diagnostic tests.
- Initiates orders for medications, laboratory studies, diagnostic testing, and treatments within scope of practice and organizational policy.
- Identifies alterations in patient health status and implements appropriate nursing and medical interventions.
- Diagnoses and manages acute and chronic health conditions across organ systems.
- Develops, implements, and evaluates individualized plans of care, considering age-specific needs.
- Provides patient and family education, counseling, and support regarding treatment plans and health management.
- Coordinates patient care in collaboration with physicians and interdisciplinary team members.
- Facilitates efficient and effective patient flow, including appropriate utilization of resources and discharge planning.
- Participates in interdisciplinary rounds, case conferences, and care planning activities.
- Ensures accurate and timely documentation of patient encounters, procedures, and care plans in accordance with policy.
- Responds to urgent or emergent patient needs and participates in on-call responsibilities as scheduled.
- Participates in quality improvement activities, including data collection, outcome analysis, and process evaluation.
- Incorporates evidence-based practice to enhance clinical outcomes and support program development.
- Engages in professional development activities and represents the organization in appropriate community or educational events.
- Performs other duties as assigned.
