Case Manager
Pay Rate: $19.95
Job Purpose: As a Case Manager at Aspire Health Partners, your job purpose to provide overall assessment and needs of chronically mental ill adults.
Job Functions:
- Providing screening, referral, emergency, crisis and case management services, participating in clinical treatment team.
- Conducting intake assessments to determine family needs, efficiently documenting all activities, conducting activities with the client and families, providing regular input to the treatment plan
- Attending community meetings to advocate for client needs, providing support and crisis intervention when necessary
- Documenting clinical records and additional program specific documents while maintaining the integrity of the client.
- Ability to use program technology to communicate effectively with staff
- On occasion, act as the authorized client transporter for necessary treatment and services.
- Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
Qualifications
- Bachelor's Degree in Social Work, Psychology or Human Services related field.
- Level II Background clearance- All Aspire Health Partners Internships and Careers require Level 2 clearance, with Aspire covering fingerprinting costs. Click https://info.flclearinghouse.com to learn more.
- Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by Aspire’s insurance company as described in the AHP Corporate Transportation Policy 3.9.1.
