Position details:

Location: Tampa, FL

Shift: 8:00Am - 4:30pm

Schedule: PRN - as needed - non-benefit eligible

Weekend Work: Occasional

On Call: No

Responsibilities:

• Provides individual and group health education counseling, health promotion activities, coordination of community resources and health screenings at various community locations.
• Supports the spiritual, emotional, social and physical health of community members working closely with FCN leadership.
• Performs Transitional Care visits as assigned in efforts to keep patients healthy at home and reduce readmissions to achieve effectiveness throughout the continuum of care.
• Participates in discharge planning, serves as a communication link between nurses and hospitalized parishioners, and performs home, hospital, and nursing home visits.
• Provides education and intentional care of the spirit to the care recipient as needed at bedside and post discharge
• Acts as a liaison for community FCN's who will begin transitional care after discharge for our partner faith communities.
• Performs other duties as assigned.

Minimum Qualifications:

• Required - Associates - Nursing; Or - Diploma - Nursing
• Required - RN (Registered Nurse)
• 2 years of Nursing experience required
