About the role:
We are seeking a Licensed Practical Nurse (LPN) to support patient-centered care delivery within Capital Health Plan's Nursing department. This role assists in providing direct patient care, health promotion, disease prevention, and patient education while working closely with providers and Registered Nurses to ensure care is coordinated, team-based, and aligned with community needs.
The LPN identifies patient needs through in-person, phone, and electronic interactions, documents care in clinical systems, supports care gap closure, and implements plans of care under the direction of a Registered Nurse or licensed provider. This role plays a key part in maintaining safe, compliant, and high-quality patient care.

We're looking for someone who has:
- Graduate of an accredited practical nursing program
- Licensed Practical Nurse (LPN) in the State of Florida
- Basic Life Support (BLS) certification
- Minimum of two years of related clinical experience preferred
- Ability to assess patient needs and provide care in person, by phone, and electronically
- Ability to document patient information accurately in healthcare systems
- Strong understanding of infection control, medication administration, and patient safety practices
- Ability to communicate clearly and professionally with patients, providers, and care teams
Highly preferred candidates also have:
- Experience working in outpatient, primary care, or ambulatory settings
- Experience identifying and addressing gaps in patient care
- Experience supporting care plans under RN or provider direction
- Strong proficiency with electronic health records (EHR) and clinical workflows
- Ability to work effectively in a collaborative, team-based care environment
